FORWARD records a number of different variables including, but not limited to: patient reported outcome measures (PROMs), comorbid conditions, medications, healthcare utilization (clinical encounters, hospitalizations, treatments/procedures), demographics, socioeconomic factors, direct and indirect medical costs, health-related quality of life scores, physical function scoring, mortality validation via National Death Index (NDI), medical event validation, and physician-confirmed diagnoses.
