How do I add hospitalizations to my questionnaire if they are newer than the dates you are asking for?

Currently the best way to keep track of these is to use a Health Diary. We include a link to the most up-to-date Health Diary in each of the emails we send out to you, but you can find one for the January 2021 – June 2021 period here.
